Q: Please Help!!!!! I am a diabetic and need to know how much insulin to take.I think I’m DKA.?
I am on sliding scale and I lost my chart Could someone please tell me where to go online to copy the humalog insulin pen sliding scale regulation chart for type 1 diabetes. I cant afford to go to the Doctor and I cant afford to miss work being stuck in a hospital. Please hurry my sugar is out of control I am still breathing OK right now but I am close to DKA which is a very bad problem. My sugar was reading high which is over 600 for about 2 days. I got it down by taking 30 units of Levemire pen and 30 units of humalog pen. I took another 13 units about 20 minutes ago because it read 384 and I’m feeling a little shaky. I haven’t known about my type 1 diabetes but for about 6 months. I’m kind of scared especially when I am confused about how much insulin to take.

A: Hi there! As a nurse I instantly noticed your question and have some concern for you. If your blood sugar continues to stay above the 400’s you need to go to the hospital. Having high glucose levels like this for long periods of time can seriously affect and impair your kidneys and peripheral circulation. Unfortunately, there is no ’standard’ sliding scale with the pen or any other insulins because the sliding scale is supposed to be designed specifically for you and your needs, based on what other insulins you take, your diet, carb intake and if you take oral antidiabetic medicines. You can certainly call your doctor’s office and have the nurse read to you the sliding scale you’ve been prescribed – just let her know you lost your list and you’d like to write it down again. But for now, if you have insurance or can find out from your doctor’s answering service the nurse ‘advice line’ – then do it.
The problem ultimately is your health, which can deteriorate more rapidly than you’d expect with blood sugars staying so high for days at a time. It might be worth it to just go on in.

Be careful not to take too much insulin – it can all catch up with you later and drop your sugars real low sometimes and make the situation worse. If you’re down to the 300’s I’d stop any more insulin intake and just keep an eye on it.

Q: Gestational Diabetes, started insulin recently, now sugars and ketones up?
I have been monitored for gestational diabetes since December. (I am 24 weeks along right now.) I had to start insulin on Feb 25th – Humulin N, 4 units at night – since my morning numbers were high (consistently over five, usually in the 6’s).
However, since starting the Humulin, almost all my number have gone up! My morning numbers have been 5.3 (not bad, but they haven’t gone down.) What is concerning me is that my numbers 1 hour after each meal have been in the nines – I have only had numbers that high twice in the three months I was monitoring, but now have had them 5 times in the last two days. Pre-meals my sugars are anywhere from 4.7 (normal for me) to in the 7s.
I am wondering if my other number could be going up because of the Humulin N? Can people have that kind of reaction? Is there just an adjustment period for the body when it starts getting insulin?
My diet hasn’t really changed, except to count my milks as carbs – so that should actually be making it easier. I am so frustrated!!! I do not want to go on the Humalog if I don’t have to, especially if there is a reaction going on here.
Anyone experience anything similar?

My ketones also went up this morning – from 5 – 15, which I haven’t had before.

Additional Details
In Canada we use a different measurement. I read (online) that it translates to 1/18 of the US measurements. So my morning sugars on insulin would be 97.2 (They want it under 90), pre-meals are 84.6 – 140’s and post-meals have gone up to into the 160’s.

I am in a small town where we have no after-hours medical, diabetes health, or even a pharmacy that is open today. I will be calling the diabetes clinic asap, but am just wondering about other people’s experiences.

Thanks Girl #2 Due! I should have said, when I mentioned I haven’t changed my diet, that I have been on a diabetic diet since December. But you have given me some great alternatives. Thanks!!

A: I have no experience with gestational diabetes, but I do have experience with diabetes in general, my husband is severe diabetic and uses Humalog and Lantus(he takes 10 units of one 15 of another 2-3 times a day. There have been days that he’s been out of insulin of one kind and we had to wait a couple days to get the next vial. When he finally gets the next vial it takes a few days to get things back in order. So your body might be adjusting. You said your diet hasn’t changed, I don’t know what you eat regularly or how, but keep sugar out as much as you can. Ask your Dr about a sweetner to replace sugar, this I would def get your Dr. advice on b/c some sweetners aren’t good while being pregnant, so you need his/her approval on what to use. Being in the US my husband’s measurements should be between 70-120, if that helps any. You’re ones are high, but they are not overly high. But I’m sure there is a difference in gestational diabetes and regular diabetes, so they probably need to be handled differently measurement wise. To help with numbers and this is just to say b/c I do now know how you are eating, but just trying to help…..look for sugar free products, orange juice can bring up your sugar(always good to have on hand in case you get too low), many juices can bring up sugar, get ‘light’ juice so you still get your juice intake, yogurt butter, whole wheat/grain bread, drink plenty of water, stay away from regular soda as much as you can(Pepsi or Mt Dew is another good thing to help bring sugar up if it ‘bottoms out’), there is sugar free diabetic candy you can have so when you need that chocolate or sweets fix, for my husband pastries knock his sugar up really high with the sugar and bread, its bad, and I have to lecture him lol. Instead of Gatorade drink the G2 version of it. These are what I could think about off the top of my head. I’m just trying to help ya out b/c it’s really hard to get sugars balanced out at first. I’m glad you are calling the diabetes center they will be wonderful help to you. Our center even provides us with glucose tabs and gel for when he bottoms out for free, which is a very scary thing.
